Solve : What is physical process of opening a connection to the internet?

I know a lot of this and how looked around, but I am finding a lot of bits and pieces. I am just missing some of the DETAILS. When I connect to my LAN (router) ------> wires-----> modem -----> ISP ----> DNS servers if need be -----> NAP if need be ----- alternate ISP (omittable) ----- alternate DNS server (omittable) -------> REMOTE server on internet.

info:
ISP is modern
router is 802.11n
default DNS is from ISP

To be speific,
How does the modem connect to the ISP? (cables, T3 line....etc.)
What are the mutiple buildings, ROUTERS, modems the connection goes through at the ISP?
If there's anything I messed up on add/modify.
via a wire to the trunk line in your area if digital cable ( which I'm assuming it is)
