A complaint can be either handwritten or typed or sent through e-mail. When a complaint is lodged in a forum/ court, it has to be accompanied by affidavit, giving information about name, address and contact number. The reasons for filing complaint should be clearly started. Xerox copies of the supportive documents/ evidences related to complaint should be enclosed. Bills and receipts of payment should be enclosed. Details of cheque should be given when the payment is made by cheque. Other evidences such as copy of advertisement, pamphlet; prospectus and terms/conditions offered by the seller should be enclosed. Bills and receipts of payment should be enclosed.
