Solve : Outlook lost reply and forward buttons?

In Outlook 2003 for a message display there are no Reply or Forward buttons.

Help points to customize the Standard Toolbar.
In the choices to Add there are no Reply or Forward.

How can I get these buttons? There are a lot of problems with this. They seem to just disappear.

I found the message format is DIFFERENT for Meeting Requests than for a standard email.
In Meeting Request Accept/Reject is used INSTEAD of Reply. There is a Forward button.
